Care Connection Center Manager
Job Description

Full-time, hourly, 32-40 hrs. a week
Hours: Flexible, Monday-Thursday
Background and reference check before hire
Probationary Period: 90 days or less from date of original hire
Reports To: CareNetworkCR’s Executive Director

CareNetworkCR connects those individuals in need to a faith community, providing a way for the Church to make significant long-term and even eternal differences in the lives of people in our community. This is accomplished through our Connection Center where referral calls are verified, and a manageable, unduplicated care plan is created through both community resources and unique church resources.

 

Mission-Driven Leadership Opportunity

 

CareNetworkCR is seeking a relational leader who loves mobilizing people and supporting teams to serve our community through basic care coordination that involves the local church. This role is ideal for someone energized by rallying a team around a mission and enabling others to succeed in helping people in need.


As Manager of the Connection Center, you will help activate and coordinate a network of churches, staff, volunteers, and community partners, ensuring that each caller receives compassionate care and a thoughtful plan of holistic support.

Key Responsibilities

 

  • Mobilize and train volunteers and Care Coordinators to serve callers with excellence and compassion. Provide coaching and performance feedback.

  • Understand and navigate social service systems.

  • Equip and encourage the team, ensuring volunteers feel supported, confident, and valued.

  • Assess needs and design supportive care plans, coordinate care plans by connecting callers with churches, community agencies, and resources.

  • Cultivate strong partnerships with local churches, ESL School, and like-minded organizations to expand care opportunities.

  • Data entry and reporting, ensuring data is clean for grants and reporting to stakeholders. Understands the data and is able to interpret the community needs.

  • Support ESL Liaisons at ESL Schools with student care plans and needs.

  • Champion the mission of CareNetworkCR through communication, presentations, and community engagement. Comfortable representing the organization publicly through public speaking or table presentations.

  • Lead daily prayer and devotion, fostering a Christ-centered culture of service and encouragement.

  • Design and review of care plans, ensuring resources are appropriate.

Skills/Qualifications

 

  • Sound leadership and ability to make quick decisions.

  • Excellent written and verbal communication skills.

  • Interpersonal skills with the ability to engage effectively with individuals from diverse backgrounds. Experience with training a team or mentoring others.

  • An understanding and commitment to a transformational pathway for callers, churches, and volunteers.

  • Ability to multi-task and work with multiple interruptions.

  • Provide sound and timely instruction, training, and communication to the team.

  • Working knowledge of Microsoft Office, Word, Excel, Outlook, Google Drive, Canva and the ability to learn and use other computer programs: UniteUs, Volgistics and Little Green Light.

  • Strong commitment to prayer and living a life worthy of your calling. Emotional maturity and resilience when working with people in crisis, ability to process difficult situations while maintaining compassion and clarity.

  • Ability to collaborate with others and work as a team.

  • Strategic thinker, problem solver.

  • Ability to navigate difficult conversations with empathy and clarity, de-escalation and conflict management skills, and maintaining healthy relational boundaries.

  • Ability to evaluate situations from micro and macro view points.

  • Ability to track program metrics and generate reports, ability to analyze trends in service requests.

  • Ability to maintain confidentiality and handle sensitive information with discretion.

  • Understands time management and skilled at prioritizing tasks and delegation.

  • Onboard partner churches with GAP Ministry needs.


Examples of Essential Work

  • Answering referral calls requesting help, listening to their story without bias, responding with compassion, discernment through prayer and question asking, exploring possible root causes of needs.

  • Lead, coach, collaborate and evaluate CCCs and CCVs as they follow scripted intake procedure while also applying appropriate “common sense” questions to gather as much information as possible.

  • Onboarding processes for volunteers, scheduling volunteer hours for call center and administrative roles.

  • Make calls to verify information i.e., employer, landlord, agencies, case worker, family members, etc.

  • Listening with care to CCC/CCV calls to assure professionalism, consistency, and encouragement.

  • Data entry to include all pertinent statistics for the Connection Center and ESL Schools. Report data to the EOS leadership team bi-weekly.

  • Initiating communication with partner churches and agencies, updating information, relationship building and servant leadership.

  • Continue to build community resource information through research and discovery.

  • Participation and promotion of CareNetworkCR’s events, social media outlets, and mission.

  • Facilitate and schedule Volunteer Development Training sessions quarterly.

  • Co-facilitate Informational Workshops monthly with the Executive Director.

  • Participate in Quarterly Celebrations with church partners and donors.

  • Public presentations of CareNetworkCR to a variety of audiences.

  • Co-develop with the team and final approval for care management plans prior to caller delivery.


Other Requirements

  • High school diploma or GED required

  • Previous Social or Case Management experience a plus

  • Call Center or consultative skills preferred

  • Office Management experience a plus

  • Agree with and be willing to uphold the Statement of Faith and the policies of CareNetworkCR.
